The patient must be an established patient of record at our practice and have been 100% consistent with bi-annual cleanings, X-rays, and exams.
The patient must wear a night guard or night retainers and bring them to appointments for confirmation of wear.
The guarantee covers only "normal" wear and tear, which we define as activities such as speaking and eating. It does not cover accidents or self-inflicted damage, such as using teeth to open objects.
The patient must maintain sufficient posterior support, defined as a minimum of four pairs of posterior chewing teeth.
The guarantee does not cover lab repair costs or office visits, each not exceeding $150. The guarantee only covers the time and skill needed for the doctor to repair or replace the damaged item.
This guarantee is limited to the repair or replacement of a single damaged or failed prosthesis (e.g., one veneer or crown) and does not cover replacing all prosthetics if only one is damaged.
